What to bring when you first come to see me regarding your Georgia Workers’ Compensation Claim

Often, clients ask what they need to bring during their first appointment. Nothing is required, and we can always obtain any information we need, but if you do have the following, please bring to your free consultation:

1) any medical records you have from this accident

2) any correspondence or Board forms you have received from your Employer or the workers’ compensation carrier

3) copy of the panel of physicians or list of doctors, if any, posted at your workplaces. It is helpful if you can look at work for this, and take a picture or get a copy of what is posted before anything is altered

4) anything else you have received or have in your possession regarding your claim
